Coaches
- Must be signed by the referee for a game being served as a suspension. Submit to PYSA office. KEEP A COPY FOR YOUR RECORDS!
Must be filled out when for each guest player used in tournament competition.
Obtain a Birth Certificate
Birth Certificates must by filed with PYSA to register your child to play. You can obtain a copy online here.
- To report a problem with, or to appeal the placement of your child on a particular team.
- If your child lives outside the boundries of PYSA jurisdiction (PISD), please click on the Release Form above before you register for PYSA.
- Request a refund from PYSA
- Submitted prior to the season. PYSA attempts to schedule around legitimate conflicts when possible.
